Immunization Information

Walk-ins: Monday, Wednesday, Friday    8:00 - 4:30

Appointments Only:  Tuesday & Thursday

If the child has private insurance (ex: Blue Cross, Coventry...),
contact their doctor's office to receive the needed vaccinations.

The vaccine we have is provided by the State for uninsured, underinsured and Healthwave Insurance.

The cost of each shot is $14.80 for children 18 and under.

Like most of our services it may be available to you at a lesser cost. 

Bring in proof of income to see if you qualify.

If your child has a Healthwave card, it will cover the cost of the immunizations.  Please bring in their Healthwave card and a copy of their immunization records at the time shots are given.

School Entry Physicals

Any new early childhood program or kindergarten student will need a school entry physical completed by a Kansas physician within 12 months prior o the first day of school.

New students under the age of 9 years who are attending a Kansas school for the time, also require a physical as described above. Documentation of the physical must be provided to the school prior to attending.

     * Varicella (chickenpox) vaccine is not required if child has had chickenpox disease and     disease is documented by a physician signature. Without physician signature, vaccine is required even if you believe your child has had chickenpox disease.

   ** Although 1 dose of varicella is requird for school attendance, 2 doses are recommended by the ACIP (Advisory Committee on Immunization Practices).

 *** All 7th - 9-th graders are required to have one dose of Tdap regardless of the interval since the last dose of Td (tetanus/diptheria). This is to improve pertussis (whooping cough) immunity due to increasing outbreaks.

****All 10th - 12th graders are required to have one dose of Tdap if more than 10 years since previous DTaP (pertussis containing vaccine). This is to improve pertussis (whoopig cough) immunity due to increasing outbreaks.
